Annandale Health & Community
Services is a 501(c)(3) non-profit corporation organized and existing
under the laws of the State of Minnesota. Membership in this
corporation is open to anyone who is at least 18 years old and donates
$5.00 or more per year to the corporation. The members elect a Board of
Directors who oversee and coordinate the operation of a nursing home, a
congregate housing facility and an assisted living.
Annandale Care Center, a 60-bed skilled nursing facility, opened its’
doors in 1982. In 1984 the Care Center added an ancillary service wing
to provide space for physical, occupational and speech therapy
inpatient and outpatient services and also added a garage. In 1991 the
garage was converted to a chapel and two detached garages were built.
In 2004 Annandale Care Center went through an extensive remodeling
including changing double occupancy rooms to single rooms and adding
private rooms. Today we have 16 single rooms 6 private
rooms and 19 double occupancy rooms.
In 1986 Heartland Express was started as an alternative transportation
method for the elderly and disabled. This transit company was sold in 2007
to River Rider and still operates in Annandale today.
Centennial Villa was built in 1987 to provide senior housing to
independent persons 55 years of age or older. This building is
connected to the Annandale Care Center by a link. Meals can be
purchased for a fee and light weekly housekeeping is provided. 22
apartments consisting of eight two bedroom, two large one bedroom and
twelve one bedroom apartments are available. Due to an overwhelming
response for assisted living services, these apartments also offer
assisted living.
Centennial Villa Assisted Living/Memory Loss was built in 2000. 7
apartments were designated as memory care apartments with a secure
entrance and special memory care services, 23 apartments; three two
bedroom, nine large one bedroom and eleven standard one bedroom
apartments are available for assisted living. In 2002, two individual
apartments within the memory care unit were converted to double
occupancy thus allowing a total of 9 persons to reside within the
memory care area.
