We are a nonprofit membership organization committed to quality health care and housing, that shape the future for the dynamic needs of the people in the Annandale Area. Annandale Health & Community Services is a campus continuum of care. It consists of the following facilities:
Skilled Nursing
Annandale Care Center is a 60 bed skilled nursing facility with rehabilitation services, beauty shop and a chapel.
Senior Housing
Centennial Villa is a 22 unit, secured apartment building offering independent and assisted living services.
Assisted Living Centennial Villa Assisted Living is a 26 unit secured apartment building offering assisted living.
Memory Care Centennial Villa Assisted Living Memory Care is 4 separate cottages, set up as a home, offering individual rooms and private baths. Mild/Moderate and Advanced Dementia Care is available.
